Summary: |
The 4th Day Program is a home visiting program offered to families that access Greater Minneapolis Crisis Nursery Services. The purpose of this position is to empower chronically stressed families by building upon family strengths, addressing identified needs and developing a community support network. These services are provided primarily on location in the family’s home and community. This specific position is tied to HPRP housing funds and will focus on helping families find and maintain stable housing. |
| General Duties and Responsibilities: |
- Facilitate the Perception of Needs Assessment with all participants.
- Perform an intake with referred families to determine eligibility for HPRP funds
- Establish a trusting relationship with families that facilitates goal planning, access and utilizing community resources
- Develop Housing Stabilization Plan (HSP) with families which will create a long term housing plan
- Collaborate with families to identify family goals and action plans to meet the stated goals
- Review the HSP plan with families quarterly
- Assist participants in understanding various parenting and child development issue that affect housing.
- Develop and maintain strong relationships with partnering agencies in the field.
- Maintain confidential files for all participants.

Education: |
Master’s degree in Social Work or related field. |
| Experience: |
Two or more years of experience working with vulnerable populations. Cross-cultural experience preferred. |

- Ability to be flexible and adaptable.
- Capacity to manage an active and complex case load
- Self Directed
- Collaborate in a team environment

| Skills: |
- Excellent interpersonal and program management skills
- Knowledge of family dynamics and child development.
- Comprehensive understanding of systems theory, strengths based philosophy and crisis intervention model.
- Familiar with community resources.
